English translation
An applicant for a licence to import, export, or transport narcotic or psychotropic substances or preparations, or chemical precursors and their preparations, shall submit an application to the competent authority stating his name and address, specifying the substances and preparations, the pharmaceutical form, the name and address of the other party, and the reason for import, export, or transport. The competent authority may add any other particulars required by any law or regulation.

على طالب الترخيص لاستيراد، أو تصدير، أو نقل المواد، أو المستحضرات المخدرة،
أو المؤثرة عقلياً ، أو السلائف الكيميائية ومستحضراتها أن يقدم طلباً إلى الجهة
المختصة، يبين فيه اسمه وعنوانه، وتحديد المواد والمستحضرات، والشكل
الصيدلاني، واسم وعنوان الطرف الآخر، وسبب الاستيراد أو التصدير أو النقل
