Article 94 of Social Insurance Law

The employer shall register its workers who are subject to the provisions of this Law with the Institution within ten days from the date they enter service. These workers may register their names if the employer refuses or delays such registration. A decision shall be issued by the Minister specifying the aforementioned registration rules, the procedures for obtaining an insurance card, the particulars it contains, the procedures to be taken if it is lost, and the rules for its use. The Institution shall give each insured person an insurance card free of charge, and a fee of (500) fils shall be paid when a replacement for a lost card is requested.
